Registration

The following information is needed for registration;

  • Students’ and parents’ name (if you are not the parent but the guardian, guardianship papers are needed)
  • Students’ date of Birth
  • Copy of students’ birth certificate
  • Copy of immunization record
  • Students’ grade
  • Previous address
  • Current address (including verification of address)
  • Previous school district (including address and phone number)

Registration forms can be mailed to you, and an appointment will be set up with a Guidance Counselor.  We will request educational records from your previous school district, however, if you have copies of educational transcripts, IEP, report cards, etc, please bring them when you meet with the Guidance Counselor in order to provide appropriate and accurate educational placement.  We request that the parent/guardian attend the appointment with your child.
