How to Obtain Your Working Papers

Contact the E-Wing Office at 533-5601 extension 1123 to obtain your working permit.  The following information is required to secure your permit:

• Complete the “Application for Employment Certificate or Transferable Work Permit.”  Be sure to have your doctor, examining physician, certified nurse practitioner, or certified registered nurse practitioner complete the SECTION D.
• Demonstration/Evidence of age.  Acceptable forms of evidence include the following:
  • Transcript of Birth Certificate
  • Baptismal certificate or transcript
  • Affidavit of parent or guardian accompanied by physician’s statement of opinion as to the age of the minor.
  • Passport
  • Driver’s License or State ID
