Daycare/Babysitter and Student Change of Address forms can be found on your parent portal for PowerSchool. Instructions on how to create a login for the parent portal can be found under the PowerSchool tab on the GJSD site:
Reminder: A new Daycare/babysitter form needs completed and approved each year for each child. The information will not transfer over from year to year.
